Compensation: $25.00/hour + Competitive BenefitsA mission-driven Alaska-based nonprofit is seeking a compassionate and resourceful Care Navigator to help individuals and families access critical health and social services. This role is ideal for someone who thrives on building trust, solving problems, and improving lives through personalized support and advocacy.The Care Navigator is a key member of the outreach team, working directly with clients to assess needs, develop care plans, and connect them with appropriate resources. This position also contributes to broader community initiatives aimed at improving health equity and reducing service gaps.Your day-to-day:Client Support & Advocacy- Conduct interviews and assessments to understand client needs, preferences, and goals.
- Develop personalized care plans that may include medical treatment, housing assistance, mental health support, and more.
- Facilitate access to services by connecting clients with providers, agencies, and programs.
- Advocate for equitable and adequate services, ensuring clients are treated with dignity and respect.
- Monitor progress, address barriers, and adjust plans as needed to meet care objectives.
- Educate clients on navigating healthcare and social service systems independently.
- Provide guidance to assist clients to obtain pertinent information, and referrals as required
Resource Development & Community Engagement- Build and maintain a comprehensive resource library of community programs, agencies, and providers.
- Identify service gaps and recommend new programs or partnerships to improve access.
- Participate in outreach efforts to promote navigation services and build awareness.
- Represent the organization at community events, workshops, and meetings.
- Assist in developing and improving internal policies and procedures related to care navigation.
- Collaborate with internal teams and external partners to enhance service delivery.
We’re looking for:- At least one year of experience in outreach, social work, or a related field (preferred).
- College degree in social work, sociology, psychology, or a related field (preferred).
- Strong communication skills to explain complex systems and build trust.
- Empathy and patience to support individuals during stressful or challenging times.
- Problem-solving skills to navigate barriers such as financial or transportation issues.
- Organizational skills to manage multiple cases and collaborate with service providers.
- Proficiency in Microsoft Word, Excel, Outlook, and Teams.
- Professional demeanor and ability to interact with the public and partner agencies.
- Willingness to travel for training and outreach events.
- Ability to maintain regular work hours and communicate effectively in English.
Physical Requirements:This role involves frequent computer use and regular interaction with clients and community partners. The work environment is typically office-based, with occasional travel to events or meetings.
