• Veterans Services Case Manager

    A faith-based organization is searching for a Case Manager to work as part of a team to assist in aiding veterans and veteran families that are experiencing homelessness or are in imminent danger of becoming homeless.
     
    Excellent communication skills both written and verbal, the ability to adapt to change while keeping a positive temperament, and the willingness to go above and beyond to provide top notch service to veterans in our community is key to being successful in this role.
     
    Candidates must be proficient with Microsoft Office Software (Word, Excel, etc.) , a valid Alaskan driver’s license is required. 
    Previous experience working with the homeless and/or at risk Veterans and families preferred.
     
    Duties & Responsibilities:
    • Provide individual case management sessions with clients at regularly scheduled intervals.
    • Develop individual housing plans for qualifying participants.
    • Document all client contact and progress, including time and date, type of contact, outcome and plan of action.
    • Provide or broker services to holistically address clients’ needs
    • Participate in the discharge process and aftercare needs of the client.
    • Provide after-hours on-call services as assigned.
    • Oversee coordination of services with other community service providers, including coordination of appointment times, facilitating activities, and resolving issues.
    • May also perform the duties of employment training specialist and/or job developer.
    • Provide outreach to the community, landlords, agencies, and veterans on the streets.
    • Other duties as assigned.

    This is a full-time position Monday -Friday with Pay starting at $24/HR depending on experience.
     
    Minimum Qualifications:
    • Associate degree in human services or related field or experience in related field
    • 2 years’ experience in delivering case management services preferred.
    • Valid Alaskan driver’s license 
    • Proficiency in Microsoft Office Software (Word, Excel, etc.)
    • Must have the ability to lift up to 40lbs 
    • Background will be viewed
    • Drug free environment
